This is the Five Wishes document i was telling you about.  i would like for you to complete it and submit it (i would imagine you'll have to upload it).  

Talk the document's ideas over with family, friends, or whomever you feel comfortable with.  This is an "advance directive," laying out your end-of-life wishes (whenever end of life may occur).  There is a Durable Power of Attorney document for your perusal at the end.  You will need to decide who will be able to make decisions for you if the need arises.

We will discuss this document in more depth on Monday.  There is no class on Wednesday, so you'll have the rest of the week to do it.  

**If you already have completed an Advance Directive, there is no need to complete the Five Wishes document.  Just submit your own Advance DIrective for the same number of points.
