PART 1:

FUNERAL SERVICE:After my death I choose to have a funeral service surrounded by family and friends. I selected this service because family is very important to me and I would like my family and friends there to see me, and to commemorate my death by gathering together and uniting as a family.

 

PRE-FUNERAL:

AGENT TO FOLLOW MY WISHES: I choose my Mother Rocio Fuller as my agent, to follow my wishes. If in any case, she is not able to attend or something were to occur, I choose my husband Roberto Maldonado as my agent to follow my wishes.

PEOPLE TO NOTIFY & HOW TO NOTIFY THEM: I would want my family to be notified. My Mother, Father, Sister, Husband, Son, Daughter and all my relatives (cousins, uncles, nieces, nephews). I want them to be notified of my death, by way of my Mother and Husband.

 

Obituary: Obituary not needed.

 

LOCATION OF DOCUMENTS:

My Mother, Rocio Fuller, has access to my financial papers, which are: Advance Health Care Directive and the California Last Will and Testament forms. My Mother knows the location of these forms.

METHOD OF DISPOSING OF BODY:

Immediate Burial at Desert Memorial Park in Cathedral City.

Where to deliver the body:

        Once I am deceased, I would like to have my body delivered to Wiefels                    Funeral Home. Please see Advanced Medical Directive.

 

ORGAN DONATIONS:

I have opted to be an organ donor. Please see my Advanced Health Care Directive.

VISITATION BEFORE DISPOSITION OF MY BODY:

I would want a visitation from my family to see me, or view my body, wherever my body is.

Extended visitation time will not be necessary.

CLOTHING FOR THE BODY:

I would like to be dressed with my favorite color peach. I would like to have a peach dress on the day of my funeral service.

ITEMS TO BE PLACED IN THE CASKET:

I would like to have 3 picture frames inside my casket. One frame will be a family photograph (My daughter, son, husband and I.) The second photograph frame will be a picture of my Mother and Step dad and the Final photograph frame will be of My sister, brother-in-law and my niece. Also, I would like the Holy Bible to be placed next to me on the right side of my casket laying close to me.

CLERGY OR PRESIDING AGENT:

I would like to have elder Omar Fong from my Church to do the speech at my funeral service. I would like a few texts read at my funeral. (John 5:28, 29)

Which company provides the most cost effective service?

Fitzhenry-Wiefels has the same options as forest lawn and the prices are much cheaper and cost effective.

What other factors might lead you to select the lesser cost effective company?

Some factors that lead me to select the lesser cost effective company is my budget. I do not want to spend a lot of money on my funeral. Also, I wouldn’t want my family to pay more money out of pocket.

I have talked to my family members about this plan. I have a savings account in Bank of America. I plan to save for half of my funeral and my Mother plans to pay the remaining half.
